To begin your online application, all you have to do is choose from one of the three options: search for Dr. Cross's name, OR simply tell us how you’re planning to use CareCredit.
We will obtain and verify information that identifies you as a new account holder. We’ll use your name, address, date of birth, Social Security, Driver’s License, and other information for this purpose.
